Form Health is an obesity Center of Excellence that helps employers provide access to science-based clinical care while controlling costs. Through a carve-out model, and in collaboration with the existing carrier and PBM, Form controls GLP-1 prescribing for obesity and ensures safe, appropriate, and efficacious use.

Patients work with an obesity doctor and Registered Dietitian who develop a personalized plan that addresses behavioral health, nutrition, and physical activity and includes a prescription for FDA-approved medication, if appropriate. Support is provided through frequent video visits and Care Team messaging along with tracking tools, classes, and resources in the Form Health app.
