Dr. Connie Hwang is the Chief Clinical Officer at Transcarent, having formerly served in the same role at Accolade prior to the merger of the two companies in April 2025. Dr. Hwang leads virtual primary care, expert medical opinion, and care management programs that are seamlessly integrated with benefits navigation. With over 20 years of population health experience, she is dedicated to continuous quality improvement to achieve optimal clinical outcomes and cost savings for consumers, employers and health plan partners.
Dr. Hwang’s previous experience includes serving as the Associate Chief Medical Officer at Twin Health, a virtual care company reversing chronic metabolic disease through AI-powered digital twin technology. Additionally, as Chief Medical Officer for the Alliance of Community Health Plans, she collaborated with clinical leaders across the country on innovations in telehealth, precision medicine and value-based care partnerships. Dr. Hwang has also served in leadership positions at Evolent Health and the National Quality Forum and continues to be an advocate for expanding access to quality care and improving health outcomes across diverse populations.
Dr. Hwang currently serves on the Patient-Centered Outcomes Research Institute Board of Governors. She is a former Robert Wood Johnson Clinical Scholar and MPH graduate from Johns Hopkins. Dr. Hwang completed her Internal Medicine residency at Thomas Jefferson University Hospital and her medical degree from Mount Sinai School of Medicine.
